Housing Stabilization
When a family faces a situation that puts them at risk for homelessness, Community Coalition for Family Wellness helps them find permanent, safe homes. Assistance may include emergency financial support, housing or mediation between tenants and landlords..
Food Support Program
A monthly incentive for individuals and families that fit eligibility requirements. Individuals and families can pick up their food incentive once per month or can arrange for a home delivery if homebound.
Bridging
Bridging is an agency that provides a “basic home set up” to those in need. The agency’s goal is to create a comfortable and functioning home. Bridging appointments can provide participants with gently-used upholstered furniture, a kitchen table and chairs, a mattress and box spring, linens, dishes, artwork, and more. Family Center staff can create a referral for you to have an appointment with bridging.
Employment Services
We help people with disabilities find employment opportunities within their communities. Our assistance helps them connect with employers and then maintain and grow those roles. Our discovery-based employment process finds the right match for everyone to a job in which they can use their skills, learn new ones, and expand their horizons.
Health Equity
Community members —particularly people of color and others who experience inequities. Community Coalition for Family Wellness can provide insight into the barriers that contribute to social determinants of health and healthcare disparities, resulting in interventions that are more responsive to community experiences, needs, and priorities. Strong partnerships with the community can also help individuals and families live a better quality of life.
Cash Assistance Program
Program designed to help homeless individual and families on a quarterly basis, determined based on eligibility. This program will provide homeless individuals and families $200 quarterly to use towards personal essentials and food support.
